8/29/2023 EnchantaVaughn Approved Behavior: Laylah was well dressed and manicured for todayâs session. The home was neat and in order when QMHP arrived. Laylah was seen preparing for session. Laylah was respectful and polite as she transitioned from her room to her therapy. Laylah was in a pleasant mood and communicated well with QMHP. Behavior: Laylah shared that she had a good day and did not get into any trouble with her mother throughout the day. Laylahâs mother appeared calm and engaged in full conversation with counselor about her day and the current emotions she was feeling. Laylah was in a good mood as evidenced by her demeanor. Intervention: QMHP went to the home for the purpose of meeting with Laylah individually. QMHP conducted session on following all directions and limiting outburst of anger at home, in session, and in the community. QMHP engaged in conversation with each family member about what has been happening in the home since the last session and if there were any concerns. QMHP listened to mother verbalize any issues or concerns in the home. QMHP reiterated with each family member about writing down things so that they have a clear review of what they are experiencing daily. QMHP will continue to assist Laylah with managing anger and frustration by reviewing and role-playing effective relaxation and coping skills. Intervention: QMHP utilized role-play to help Laylah understand the importance of following all directions and limiting outburst of anger. QMHP implemented an activity to increase self-esteem by receiving positive comments from mom and Laylah. The activity was also completed to hear positive comments about themselves. Each family member had to give positive comments about each other. Each family member had to take turns sitting in a chair, in which each family member has to give positive comments to the family member that is sitting in the chair. QMHP utilized active listening skills while Laylah excitedly discussed her feelings of being in service and talking about her problems. QMHP processed Laylah feelings and offered choices of what to do when faced with conflict or challenged in the home. Response: Laylah displayed a welcoming demeanor when greeting her counselor in the home setting. Laylah exhibited a positive attitude toward mother and sibling. Laylah verbalizes positive feelings about herself. Laylah smiled and hugged QMHP. Laylah interacted well with her family, demonstrating good team building and relationship building skills. Laylah displayed good manners in the session. Laylah expresses good statements about her day saying she was doing her work, using her manners, and being fair to others. Laylah is more confident at home, as evidenced by assertive and proud tone. Laylah have been doing a better job controlling impulsive and hyperactive behaviors. Laylah transitions smoothly and safely to next activity, displaying appropriate behaviors in the home. Response: Laylah did an excellent job by showing interest in the session by asking appropriate questions. Laylah showed gratitude when the counselor complimented her behavior. Laylah shared that she tries to remain in a more positive state of mind now that she understands her mental illness. Laylah expressed a cheerful spirit. Laylah was attentive during demonstrations of positive behavior as evidenced by giving eye contact and asking questions while examples were being provided. Laylah was attentive and engaging in conversation throughout the session. Laylah communicated that she is working to change her behaviors and reactions to certain situations. Laylah continues to make progress with ISP objectives.
